Registration

Welcome to the Pagewood Botany Football Club.

2010 Registration closed.

Your registration fee includes a new jersey, shorts and socks, you will need to supply your own boots & shinpads for the season. They are available through Rebel Sport Eastgardens and as a member of PBFC you will receive a 5% discount on any full price items purchased at Rebel Sport throughout the year. Please mention our Club ID # 33927 to receive this discount.

All registration takes place at Jellicoe Park, Pagewood.  The age you are turning in 2011 will be the age group in which you register.

Juniors: Under 11's to U18's - For players born 1993 to 2000 Registration

Girls:    Under 11's to U18's -  For players born 1993 to 2000 Registration

FEES

The fees for the 2011 season are as follows.

Please note there is a $30 discount for each sibling. Senior Student Players receive a $40 discount

PAYMENT

Payment can be made via cheque, eftpos or credit card (Not American Express) on the day.

REGISTRATION PROCESS

JUNIORS

For new and returning players you will need to bring the following:

1) A completed PBFC Registration Form ( download PDF-Junior )

2) A completed PBFC Code of Conduct Form ( click here ) 

3) A completed Accident Support Program form ( click here )

4)A Photocopy of birth certificate or passport ( New players Only )

All new players are required to attend registration days in person to have their photo taken and complete required paperwork.  Original birth certificate or passport needs to be sighted (visit: http://www.bdm.nsw.gov.au/ for information on obtaining copies of birth certificates for children born in NSW).

SENIORS

For new and returning players, you will need to bring the following:

4) A photocopy of drivers licence,birth certificate,passport or proof of age card ( New players Only )

INSURANCE

Student Rate:**

a) All school children
b) Full time students with no employment or engaged in part time employment
c) Includes Personal Accident & Public Liability Insurance

** Does not include Income Protection Cover

Employed Rate:

a) Employed Persons
b) Unemployed over 18 years of age
c) Persons who normally attend to domestic duties
d) Includes Personal Accident, Public Liability Insurance and Income Protection Cover

Important Notice:

Employed Rate or Student Rate of Insurance must be determined at the time of registration. There will be no student rebates at a later date. Persons eligible for the Student Rate, who are engaged in part time employment and who wish to be covered for Income Protection will need to clearly request the Employed Rate of Insurance at the time of registration so that clubs indicate “employed” in the player registration system. Adults in full time study or engaged in part time employment not requiring Income Protection Cover can apply for the Student Rate at the time of registration, subject to completion of the prescribed student declaration form.

Registering with Pagewood Botany Football Club means that you have read and accepted our Codes of Conduct.

What does your registration pay for?

Your registration covers:

·     Insurance
·     Relevant affiliation fees
·     Council ground hire
·     Electricity for ground lighting, canteen etc
·     Maintenance and other fees relating to the grounds
·     Purchasing of equipment such as balls, uniforms, trophies, training equipment etc

Registration fees do not cover our outgoings alone. We rely on fundraising, canteen sales (BBQ) and sponsorship to help cover our costs.
